Systemic Problem

Harm, discrimination, and systemic failures are often invisible in health systems. Reporting mechanisms are fragmented, non-transparent, and weakly connected to institutional learning. Patients who experience problems often lack effective recourse.

Existing Landscape

Complaints systems, ombuds offices, and safety reporting tools exist but are rarely interoperable, patient-centered, or designed to support systemic learning.

What We Build

CareWatch is a patient-led reporting and learning infrastructure that enables structured incident reporting; aggregates signals across institutions; supports institutional accountability; and protects contributors.

Our Role

We design the governance, data models, and participatory evaluation frameworks for CareWatch.

Evaluation Strategy

We evaluate visibility of harms, response times, quality of institutional responses, and learning loops.

Opportunities

We welcome collaborations with hospitals, ombuds offices, and patient organizations interested in redesigning accountability systems.
