SwissNeuroRehab

Systemic Problem

Neurorehabilitation pathways are fragmented across institutions, cantons, and disciplines. Outcomes, patient trajectories, and service availability are poorly visible at the system level, limiting learning, coordination, and equity.

Existing Landscape

Data are siloed across hospitals, rehabilitation centers, insurers, and research projects. Patient-reported outcome measures (PROMs) and experience measures (PREMs) are inconsistently collected, and interoperability between systems is weak.

What We Build

SwissNeuroRehab is a federated national infrastructure that connects clinical, functional, and patient-reported data; enables longitudinal pathway analysis; supports care coordination across institutions; and enables public-interest digital twins for system-level learning.

Our Role

Convivens Lab leads the infrastructural design, governance models, and participatory evaluation frameworks for SwissNeuroRehab.

Evaluation Strategy

We assess continuity of care, functional outcomes, equity of access across cantons, and institutional learning capacity.
